What fertility is, biologically
Fertility is the capacity to produce a healthy gamete (egg or sperm), fertilize, implant, and carry to term. Each of those steps depends on different biology. The standard fertility workup measures some of it — hormone panels, semen analysis, ovarian reserve — but a meaningful share of what determines IVF success isn’t on the standard workup at all.
What’s also true: the window in which you can shape these variables is short. Egg final maturation happens over ~90 days before retrieval. Sperm development takes ~74 days. The inputs you change today are reflected in the gametes used in your cycle. This is why the 12-week pre-IVF window matters so much.
The fertility hierarchy is upside down
Most fertility advice prioritizes the wrong inputs. The substances most coaches obsess over — caffeine, alcohol, supplements, restrictive food rules — sit near the bottom of the evidence hierarchy. The variables with the strongest prognostic weight are rarely named.
What the evidence actually prioritizes:
- Chronic loneliness and emotional isolation
- Daily emotional state on waking
- Sleep quality and architecture
- Connection with your partner
- Parasympathetic activation — joy, sex without agenda, the absence of fight-or-flight
- Mediterranean dietary pattern (pattern, not individual food rules)
- Then, and only then, specific substances at heavy doses (heavy alcohol, very high caffeine)
The woman white-knuckling perfect food while skipping her sister’s birthday is losing on every line that matters. The same is true for him: cutting alcohol while sleeping at midnight and not sharing what he’s going through.
Both partners. Sperm contributes half the embryo’s DNA
Optimizing only one partner is optimizing half the equation. About 30-40% of unexplained IVF cycle failures involve elevated sperm DNA fragmentation that the standard semen analysis misses. Couples who do this work together — calmly, consistently — outperform couples where one partner is doing “everything” and the other is along for the ride.
The compound effect
One unaddressed input cancels two well-managed ones. Three unaddressed inputs cancel five. Each factor that affects egg or sperm quality has prognostic weight on its own. They don’t add — they converge. The diet won’t protect a cycle that’s also fighting chronic cortisol, social withdrawal, and 11 p.m. screens.
