The problem shows up later: in how the embryo develops, whether it implants, and whether a pregnancy holds. This is why a man can have a “perfect” semen analysis and a couple can still face an unexplained failed cycle or recurrent loss. The test that looks at the cargo, not the packaging, is the sperm DNA fragmentation test.

What the DFI test measures (and how it’s different)
Sperm DNA fragmentation testing gives you a DFI — DNA Fragmentation Index — the percentage of sperm in a sample whose DNA is broken or damaged. If your result is 24%, that means roughly a quarter of the sperm carry fragmented DNA.
Where a semen analysis answers “how many, how fast, what shape,” the DFI answers a different question entirely: “how intact is the genetic material these sperm are carrying into the embryo?” They’re complementary tests, not competing ones. A man can pass one and not the other — which is precisely the scenario that sends couples looking for answers after a cycle that “should have worked.”
The main types of DFI test
There isn’t one single sperm DNA fragmentation test — there are several assays, and they report results a little differently. You don’t need to memorize the chemistry, but it helps to recognize the names your clinic might mention:
- SCSA (Sperm Chromatin Structure Assay). One of the most established and widely referenced tests, run on a large number of sperm and reported as a straightforward DFI percentage. Much of the commonly cited threshold research comes from SCSA.
- TUNEL. Directly labels the actual breaks in the DNA strands. Also reported as a percentage of affected sperm.
- Comet assay. A sensitive lab method that measures the extent of DNA breakage in individual sperm.
- SCD / Halosperm (sperm chromatin dispersion). A more accessible test that produces a “halo” around sperm with intact DNA; sperm without a halo are counted as fragmented.
The practical takeaway: because these assays measure slightly different things, a “high” cutoff on one isn’t identical to another. That’s not a reason to overthink it — it’s a reason to have whichever result you get read by the clinician who ordered it, against the reference range for that specific lab.
What a sperm DNA fragmentation test typically costs
This is one of the most common questions, and the honest answer is: it varies more than most tests, because it’s often ordered outside routine fertility panels.
In the United States, a DFI test commonly runs somewhere in the range of $100 to $600 out of pocket, depending on the lab, the assay, and whether it’s bundled with other testing. Some specialty andrology labs sit at the higher end; some clinics offer it as part of a broader male workup at a lower incremental cost.
A few things that push the number around:
- The assay. SCSA and TUNEL may be priced differently than an SCD/Halo test.
- Where it’s run. A hospital-affiliated lab, a fertility clinic, and a mail-in specialty lab can all quote different prices.
- Insurance. This is the big one — sperm DNA fragmentation testing is frequently not covered, because some insurers still classify it as elective or investigational. It’s worth asking your clinic for the self-pay price and checking your coverage before you assume either way.

You don’t need to be a scientist to request it. A simple, direct ask works:
“We’d like to look at sperm DNA fragmentation — a DFI test — in addition to the standard semen analysis. Can we order that, and which assay do you use?”
A few practical notes that tend to come up:
- Abstinence window matters. Many labs ask for a specific abstinence period before the sample (often around 2–3 days). Ask what your lab prefers, because it can affect the result.
- Timing. If you’re actively working on lifestyle changes, some couples choose to test, work the window, and consider re-testing — because, again, the number can move.

How to read your result: normal, moderate, and high DFI
When your result comes back, it’s a single percentage, and the natural question is “is that bad?” Here’s a general reference many labs use — with the important caveat that exact cutoffs depend on the assay and the lab:

A number on a page is a starting point for a conversation, not a diagnosis you interpret alone.
What a high DFI number actually means — and why it’s not a life sentence
If your number comes back high, it’s easy to read it as a closed door. It isn’t. And this is the part I most want couples to sit with.
Elevated sperm DNA fragmentation is associated with tougher outcomes — lower fertilization and implantation, and higher miscarriage rates in systematic reviews of the research (Robinson et al., 2012).1 But it’s a risk factor, not a guarantee — and unlike your age or your ovarian reserve, it has an expiration date.
Why 74 days? A quick clarification
Spermatogenesis — the full production of a single sperm cell, from early stem cell to mature sperm — takes roughly 74 days in men. That specific figure comes from a classic 1963 study in Science by Heller and Clermont, who tracked labeled cells through the process and estimated the cycle at about 74 days (Heller & Clermont, 1963).2 It’s still the number cited today, though it varies somewhat from person to person.
After those ~74 days of production in the testis, sperm spend roughly another two weeks maturing and traveling through the epididymis before ejaculation — which is why you’ll also see this window described as about 90 days, or three months, start to finish. Both numbers describe the same reality: sperm are made fresh, on a cycle, over roughly a season.
That’s the whole reason the timing is hopeful: the sperm in a sample today reflect the conditions of the last two to three months, and the sperm maturing right now are still being shaped by what happens next.
Much of what drives DNA fragmentation is oxidative stress — and oxidative stress responds to everyday inputs. The factors most consistently linked in the research include heat exposure, smoking and vaping, heavy alcohol, poor sleep, chronic stress, and certain metabolic conditions. Some medical contributors, like a varicocele, are managed by a physician where relevant. I’m not going to hand you a supplement protocol from a blog post — the right inputs depend on the person, and anything you take should be confirmed with your own clinician — but the headline is genuinely hopeful: a high number now is often a starting point with a runway, not a fixed trait.
That’s the whole reason the pre-cycle window matters. If you test, find an elevated number, and give the next 74–90 days the right conditions, you’re working with the biology instead of against the clock. - After a failed IVF or ICSI cycle, especially an unexplained one
- With recurrent miscarriage or recurrent pregnancy loss
- With unexplained infertility despite a normal semen analysis
- When the male partner is over 40
- When there’s a known or suspected varicocele, or a lifestyle history (heat, smoking, etc.) that raises the question
- Simply wanting the complete picture before a first cycle, rather than after one that didn’t work
