If you're reading this at 2 a.m. after a cycle that didn't work, I want to say something before we talk about food: nothing you ate — or didn't eat — caused that outcome. IVF fails for reasons that are often invisible even to your care team, and no dietary pattern on earth guarantees a different result next time. What nutrition can do is more modest and more hopeful: it can shape the environment in which your next set of eggs matures and your partner's next generation of sperm is assembled. That environment is being built right now, whether or not anyone is paying attention to it. This guide is about paying attention to it — calmly, together, and with the evidence in view.
What to eat before IVF: the short version
When researchers study diet and fertility, the pattern that keeps surfacing is Mediterranean-style eating. Not a rigid meal plan — a pattern. Its core principles are simple:
- Mostly plants: vegetables and fruit at most meals, plus legumes (beans, lentils, chickpeas) several times a week
- Whole grains instead of refined ones: oats, brown rice, barley, whole-grain bread and pasta
- Fish and seafood as the featured animal proteins, roughly two to three times a week
- Extra-virgin olive oil as the main added fat, with nuts and seeds most days
- Less red and processed meat, and meals built around plants and fish rather than around meat
- Little added sugar and few ultra-processed foods — the packaged snacks, sweetened drinks, and refined convenience foods that quietly displace everything above
A comprehensive review by Gaskins and Chavarro at Harvard concluded that dietary patterns built on whole grains, vegetables, fish, and unsaturated fats — the Mediterranean template — are consistently associated with better fertility in women and better semen quality in men, while trans fats, added sugars, and heavy red and processed meat intake tend to sit on the unfavorable side of the ledger (Gaskins & Chavarro, 2018). Why do the 90 days before IVF matter for both partners?
This is the piece almost nobody explains in the consult room, and it changes how couples think about the weeks between cycles.
Her side of the window. The eggs retrieved in your next cycle are not created that month. The follicles that will respond to stimulation have been moving through their final growth phases for months beforehand — classic work on primate folliculogenesis shows the last stages of follicle development unfold over several months, with the final gonadotropin-sensitive stretch spanning the weeks directly before ovulation or retrieval (Gougeon, 1996). During that window, the egg sits inside follicular fluid whose composition — nutrients, glucose, antioxidants, B-vitamins — reflects, in part, what its owner has been eating. Diet doesn't change how many eggs you have; it may influence the environment in which the ones already queued up complete their maturation. His side of the window. Sperm are manufactured continuously, from scratch, in a production cycle that takes roughly 74 days from stem cell to mature sperm, plus transit time (Neto et al., 2016). That means the sperm sample used at your next retrieval will be built almost entirely inside this 90-day window — under whatever nutritional, oxidative, and metabolic conditions his body provides during those weeks. This is why male-side nutrition is not a courtesy gesture; it is a biologically distinct project running on the same timeline. Does the Mediterranean diet improve IVF success?
Here is where I owe you precision instead of hype. The honest answer: the evidence is observational, consistent, and encouraging — and it cannot prove cause and effect.
The study that first put this on the map followed 161 couples through IVF/ICSI in Rotterdam. Couples with high adherence to a Mediterranean-style pattern — high in vegetable oils, vegetables, fish, and legumes, low in snack foods — had higher odds of achieving pregnancy (odds ratio 1.4), and the pattern tracked with higher B6 and folate levels in blood and in the follicular fluid surrounding the egg (Vujkovic et al., 2010). Notably, the diet score was measured for the couple — both partners.
The strongest single study in women came from Athens: 244 non-obese women undergoing a first IVF cycle were scored on Mediterranean diet adherence before treatment. Women in the lowest adherence tertile had clinical pregnancy rates of 29.1% versus 50.0% in the highest tertile, and live birth rates of 26.6% versus 48.8%. The association was concentrated in women under 35 and was not seen in women 35 and older (Karayiannis et al., 2018).
Why might the association weaken with age? Nobody knows for certain. It may be that age-related changes in egg biology become the dominant factor, or that a three-month dietary window is simply too short to move the needle against them. If you're over 35, I read this evidence as "diet still makes sense as foundation-building, with humbler expectations" — not "diet doesn't matter for you."
Why does Mediterranean-style eating map onto fertility biology?
Three mechanisms show up again and again in the literature, and they explain why this particular pattern — rather than any single superfood — keeps correlating with reproductive outcomes.
1. Insulin and blood sugar. The ovary listens to insulin. Chronically elevated insulin — driven by refined carbohydrates, sugary drinks, and the blood-sugar rollercoaster of ultra-processed eating — can disrupt ovulatory signaling and alter the hormonal environment of the growing follicle. Reviews of diet and female fertility consistently find that slower-release carbohydrates (whole grains, legumes) and lower glycemic load associate with better ovulatory function (Chiu et al., 2018). 2. Oxidative stress. Both eggs and sperm are exquisitely vulnerable to oxidative damage — sperm especially, because they carry little repair machinery and their membranes are rich in fragile polyunsaturated fats. A diet dense in vegetables, fruit, nuts, and olive oil supplies a steady stream of dietary antioxidants and polyphenols; a diet dense in ultra-processed food tends to increase oxidative load while crowding those protectors out (Salas-Huetos et al., 2017).
3. Inflammation. Low-grade chronic inflammation is increasingly implicated in implantation and early pregnancy biology. Mediterranean-style eating is one of the best-documented anti-inflammatory dietary patterns in all of nutrition science, while patterns heavy in processed meat, refined grains, and added sugar push in the opposite direction (Gaskins & Chavarro, 2018).
Notice what these three have in common: none is about a magic nutrient. They are about the background chemistry your gametes develop in, meal after meal, for weeks. That is why the pattern matters more than any individual food on the list.
What should she eat before egg retrieval?
Within the Mediterranean template, a few emphases have particular support on the female side:
Folate-rich foods, alongside your prescribed folic acid. Leafy greens, legumes, and citrus contribute dietary folate, and in the Rotterdam cohort, Mediterranean-pattern eating correlated with higher folate and B6 not just in blood but in follicular fluid — the egg's immediate bathwater (Vujkovic et al., 2010). Food complements, but does not replace, the supplement your clinic has prescribed.
Whole grains and slow carbohydrates. In the Harvard-affiliated EARTH cohort and related literature reviewed by Chiu and colleagues, higher whole-grain intake among women undergoing IVF was associated with higher probability of live birth, possibly through effects on the endometrium; refined carbohydrates and high glycemic load sat on the unfavorable side (Chiu et al., 2018).
Fish and omega-3 fats. The same review found seafood and omega-3 fatty acid intake generally associated with favorable fertility treatment outcomes. Choose lower-mercury options — salmon, sardines, trout — and follow your own clinician's guidance on species and frequency during treatment.
Full-fat versus low-fat dairy? The evidence here is genuinely mixed. Some ovulatory-infertility data favor full-fat dairy, but IVF-specific findings are inconsistent, and reviews stop short of a firm recommendation (Gaskins & Chavarro, 2018). My honest read: dairy is not a make-or-break decision. Pick the form you enjoy and spend your effort on vegetables, whole grains, and fish, where the signal is stronger.
One thing she should not do: restrict aggressively. The pre-IVF window is a terrible time for crash dieting. The follicle is an energy-hungry structure, and severe caloric restriction is its own stressor. This is about composition, not deprivation.
What should he eat in the 74 days before retrieval?
The male side of the table has, if anything, tidier evidence — because semen parameters are measurable in a way egg quality is not.
In the Athens cohort's male arm, 225 men from couples attending a fertility clinic were scored on Mediterranean diet adherence. Men in the lowest tertile were roughly two-and-a-half to three times more likely to have sperm concentration, total count, and motility below WHO reference values than men in the highest tertile — 47.4% versus 16.7% below reference for concentration, 65.8% versus 31.8% for total motility (Karayiannis et al., 2017). Cross-sectional, yes — but the direction agrees with a systematic review of observational studies, which concluded that diets rich in fish, vegetables, fruit, whole grains, and low-fat dairy consistently associate with better semen quality, while processed meat, sweets, and sugared drinks associate with worse (Salas-Huetos et al., 2017).
And on the male side we even have a randomized controlled trial. In the FERTINUTS trial, 119 healthy young men eating a Western-style diet were randomized to add 60 grams of mixed nuts daily (walnuts, almonds, hazelnuts) for 14 weeks — one full sperm production cycle. The nut group showed significant improvements in total sperm count, vitality, motility, and morphology, along with reduced sperm DNA fragmentation, compared with controls (Salas-Huetos et al., 2018). One trial in healthy men doesn't prove nuts fix subfertility — but it is rare, welcome experimental evidence that food choices can shift measurable sperm outcomes within a single 74-day build cycle.
A note that matters to many couples reading this after a failed cycle: a normal semen analysis does not close the book on male-side contribution. Standard parameters say little about DNA integrity inside the sperm head — a separate question I unpack in why a normal semen analysis can coexist with a failed cycle.

What does a realistic week of eating before IVF look like?
Patterns beat plans, so I'll give you rhythms rather than recipes. A week that quietly hits the Mediterranean template looks like this:
- Anchor breakfast once, repeat it. Oats with walnuts, berries, and yogurt — or eggs with vegetables and whole-grain toast. Decision-free mornings protect the rest of the day.
- Two fish dinners. Pick a day (say, Monday and Thursday) rather than an intention. Tinned sardines and frozen salmon count fully; nothing about this requires a fish market.
- Two legume dinners. Lentil soup, chickpea curry, a bean chili. These displace meat-centered meals without anyone announcing a dietary regime.
- A handful of nuts daily, for both of you. Put the jar somewhere visible. His 74-day project and her follicular window are both served by the same 30–60 grams.
- Vegetables at two meals a day, minimum. Frozen vegetables are nutritionally legitimate. Roast a tray twice a week and repurpose it.
- Olive oil as the default fat. One decision, made once, at the grocery store.
- One shared weekend meal that feels like pleasure, not protocol. Sustainability is a nutrient too. A pattern you can hold for 90 days beats a perfect week you abandon by day ten.
Notice that the two of you are eating from the same pot. That matters practically — one grocery list, one cooking effort — and it matters relationally. In the Rotterdam data, it was the couple's adherence that tracked with pregnancy odds (Vujkovic et al., 2010). Preparation this way is something you do together, not something one partner performs while the other watches.
What should you limit before IVF?
Sugar-sweetened drinks are the clearest single target. In a prospective study of 340 women undergoing IVF, sugared soda intake was associated with fewer total and mature oocytes retrieved, fewer top-quality embryos, and a 12–16 percentage point lower adjusted live birth rate compared with women who drank none — while coffee, caffeine, and diet sodas showed no such association (Machtinger et al., 2017). Outside the IVF context, a large preconception cohort found that both female and male sugar-sweetened beverage intake was associated with reduced fecundability — roughly 20–25% lower monthly conception probability at seven or more servings a week (Hatch et al., 2018). Observational again — but note that the male association existed independently. His soda habit is not neutral.
Trans and industrially hardened fats. In the Nurses' Health Study II, each 2% of energy from trans fat consumed in place of carbohydrate was associated with a 73% higher risk of ovulatory infertility (Chavarro et al., 2007). Trans fats are now heavily restricted in many countries, but they linger in some fried, packaged, and bakery products — one more reason the "fewer ultra-processed foods" principle does a lot of quiet work.
Processed meat. Repeatedly associated with poorer semen quality in the male literature (Salas-Huetos et al., 2017). It doesn't need to vanish; it needs to stop being a daily default.
Alcohol and caffeine deserve one honest sentence each: the IVF-specific evidence for moderate caffeine is largely reassuring, the alcohol evidence is mixed enough that many clinics advise minimizing it during treatment — and your own clinic's guidance on both supersedes anything written here.
What I'd ask you not to do is turn this list into a purity contest. One birthday cake in your 90 days will not undo the window. Chronic patterns are the exposure the studies measure; single meals are noise.
When is food not the whole answer?
Almost always, in some sense — and it's important to say so plainly. Diet is one input into a system with many. It cannot correct a blocked tube, dissolve a fibroid, resolve severe male-factor infertility, or turn back ovarian aging. The Athens data themselves showed no diet association in women over 35 — a finding I'd rather tell you about than hide (Karayiannis et al., 2018).
Food also can't substitute for diagnosis. If cycles keep failing without explanation, the productive questions are diagnostic ones — about embryo quality, implantation factors, sperm DNA integrity, endometrial receptivity — asked with your reproductive endocrinologist. Nutrition prepares the ground; it does not replace the investigation.
And a word for the partner reading this with a knot in their stomach: if food tracking is becoming a source of anxiety, guilt, or conflict, that cost is real and it counts. Stress does not need another supervisor. The goal of the 90-day window is a calmer, better-provisioned run-up to your cycle — not a second full-time job.
